Have installed a few evohomes on combi boilers, but last one was for S plan
BDR91 that operates HW MV, reacts to HW heat demand from controller (motorised valve opens)

Have I got a faulty BDR91?

The S Plan configuration is different to a Combi boiler setup.

I assume you are getting the heating motorised zone valve opening on hot water demand? This happens when you have a 'wireless relay box' setup under 'boiler control'. With S Plan you don't configure the system with a 'boiler control' relay.

BDR responds to hot water heat demand but the green light on it dies not cone on but HW MV operates

No green light on BDR91 but relay still operates motorised zone valve? Faulty green LED?
